How to build a claim-evidence-source note system
Research note system for source IDs, quotations, paraphrases, claim maps, limits, contradictions, verification status, and drafting without lost attribution.

A reliable note system answers three questions at any moment: Where did this come from? What does the source support? Which sentence in the draft uses it?
The tool can be a spreadsheet, database, paper cards, or plain text. Consistent fields matter more than software.
Step 1: Create a source register
Assign a short ID as soon as a source enters the project. Do not wait until the bibliography stage.

Save publication details, page range, edition, and rights information while the source is open. URLs alone are fragile identifiers.
Step 2: Use one note template
Create one note for one proposed claim. Give it a unique note ID.

The distinction between source material and inference is the system's main safeguard. Never blend them in one unmarked paragraph.
Step 3: Capture exact language accurately
When copying words, use quotation marks immediately and preserve spelling, punctuation, and omissions. Record the page, paragraph, timestamp, or archival item. If a transcript is machine-generated, label it and check the audio before quoting.
Purdue's one-page overview of plagiarism and source tracking recommends recording sources during research and using reference numbers beside notes. It also warns that failed paraphrasing and misattribution can produce unintentional plagiarism.
Do not paste a passage into ordinary notes with the plan to rewrite it later. The visual boundary may disappear as the draft moves.
Step 4: Write a true paraphrase
Read the source, close or cover it, and state the relevant idea from understanding. Then compare both versions for accuracy, distinctive phrasing, scope, and emphasis. Add attribution even though the wording is yours.
A paraphrase is not a sentence with several synonyms exchanged. It should reflect your essay's focus while preserving the source's meaning and qualification.
Store the exact passage under the paraphrase so a later fact check can compare them.
Step 5: Add context and limits
Every evidence note should state what surrounds the extracted point:
- who or what was studied;
- time and place;
- sample or document type;
- definitions used;
- conditions and exclusions;
- whether the source reports, argues, estimates, or recalls;
- what contrary material appears nearby.
If a study reports association, record that it does not alone establish cause. If a speaker describes their experience, do not silently turn it into a population-wide claim.
Step 6: Map notes to claims
Create a second table for the essay's reasoning:

This map exposes claims supported only by rhetoric, or facts supported by multiple sources that all derive from one origin.
Step 7: Tag note status
Use a small controlled vocabulary:
- confirmed: original source checked and claim fit verified;
- partial: supports only part of the sentence;
- disputed: strong sources conflict;
- secondary lead: points toward an original not yet checked;
- replace: unsuitable authority or scope;
- permission pending: use depends on rights or consent.
Do not use color alone. Text labels remain understandable when printed, exported, or shared accessibly.
Step 8: Draft with source IDs visible
Write a rough sentence followed by its source and note IDs. Keep markers until fact-checking is complete.

After drafting, reopen the original source and check the sentence, not just the note. Confirm names, dates, quantities, verbs, context, and attribution.
Step 9: Preserve versions and decisions
Keep dated snapshots of the register and claim map. When a source changes the thesis, record the decision. If a claim is removed, retain the research note so it does not return later without its contradiction.
Store sensitive interviews and personal data according to consent, law, and organizational policy. A thorough note system should not become an unnecessary privacy risk.
Common questions
Does every note need a quotation?
No. Use the mode appropriate to the evidence, but retain enough exact support and location to verify your wording.
Can one note support several claims?
It can be referenced by several claim IDs, but split notes when different passages, contexts, or limits apply.
Should source IDs appear in the published essay?
Usually not. They are production markers. Replace them with the required citation or attribution after verification.
What is the minimum useful system?
A source register, atomic notes with locators, a claim map, status labels, and a final check against originals.
